Nerandomilast is a phosphodiesterase 4 (PDE4) inhibitor that preferentially inhibits the PDE4B subtype. It has one chiral center and is the R-stereoisomer. On October 7, 2025, nerandomilast was approved as the first new treatment for idiopathic pulmonary fibrosis in more than a decade. Nerandomilast exerts both anti-fibrotic and immunomodulatory effects as PDE4 inhibition elevates intracellular cAMP levels and reduces the expression of pro-fibrotic growth factors and inflammatory cytokines, which are overexpressed in IPF. Compared to pan-PDE4 inhibitors, selective PDE4B inhibitors are associated with a lower potential for gastrointestinal adverse events. On December 19, 2025, the U.S. FDA approved nerandomilast tablets for the treatment of progressive pulmonary fibrosis (PPF) in adults. In the Phase 3 FIBRONEER-ILD trial, nerandomilast led to a smaller decline in forced vital capacity (FVC) than placebo over 52 weeks.
